Dystonia Survey 2025

Your Voice Matters! 

If you or a loved one is living with Dystonia, we invite you to participate in our survey!  

Our goal is to gather key information that will help us advocate for better support, programs, and initiatives for the dystonia community across Canada. By participating, you are playing a vital role in identifying the needs of the dystonia community, advocating for essential resources, and driving meaningful change. The more voices we hear, the stronger our advocacy becomes - ensuring resources go where they’re needed most. Take a few moments to share your experiences and help make a real difference!

The survey will take approximately 20 minutes to complete. Questions will ask about your experiences with dystonia treatments, your ability to access care and support, as well as the impact of dystonia on your quality of life. Your individual responses will remain anonymous and confidential. Click here to view our privacy policy and data protection guidelines. Upon completion, you can enter a draw for a $300 prize — one winner in each province.

Prizing and Contest Rules

Contest Rules & Liability:

1. Eligibility
This Contest is open to legal residents of Canada over eighteen (18) years of age OR Legal age of Majority in your province. VOID WHERE PROHIBITED BY LAW. Employees of the Dystonia Medical Research Foundation Canada (“DMRFC”) and their respective parents, affiliates, subsidiaries, advertising, and promotional agencies, and the immediate family members and those with whom they are domiciled each are not eligible.

2. How to Enter
The Entrant can register for the contest by actively participating once in the DMRF Canada Survey online and completing the contest entry form via survey monkey between March 12, 2025 and June 15, 2025.
By entering the Contest, the Entrant consents to the release of his/her name in the event they are selected to be one of the winners. The Entrant declares to have complied with the Contest Rules. Further, the winners must conform to all directions given by the Contest organizers or his/her participation in the Contest will be denied.

3. Contest Period
Entrants must participate in the 2025 DMRF Canada Survey during the contest period March 12, 2025 and June 15, 2025.

4. Prizes
There are prizes of $300 each. These prizes are not refundable nor transferable and cannot be extended. Prizing, once issued in the name of the winner are not transferable or refundable. Prize must be accepted as awarded and no cash equivalent is available. NO SUBSTITUTION OF PRIZE IS PERMITTED EXCEPT BY DMRFC. DMRFC RESERVES THE RIGHT TO SUBSTITUTE ANY PRIZES WITH ANOTHER PRIZE OF EQUAL OR GREATER VALUE.

5. Winner Selection and Qualification
The winners will be chosen at random at the end of the contest period. The prize winners will be messaged by email and/or phone and may be required to complete and return a release form in order to redeem his or her prize via email. Return of email/phone notification as undeliverable/unanswerable after 3 attempts or return of the mailed prize as undeliverable will result in forfeit of prize. By entering this Contest the entrants automatically agree to accept and abide by these General Rules and Regulations.

6. General Terms and Conditions
Contest entrants agree to abide by the terms of these General Rules and Regulations. This Contest is governed by the laws of Canada. All federal and local laws and regulations apply. Return of any prize/prize notification as undeliverable may result in disqualification and selection of an alternate winner. By entering, participants release and hold harmless the Contest Sponsors, DMRFC and their respective parents, subsidiaries, affiliates, directors, officers, and employees (collectively, “Contest Entities”) from any and all liability for any injuries, loss or damage of any kind arising from or in connection with the Contest, or the use or acceptance of any prize won. Restrictions, conditions, and limitations apply.

BY ENTERING THIS CONTEST, YOU AGREE TO BE BOUND BY THESE GENERAL RULES AND REGULATIONS. The rules will be posted at all times at https://dystoniacanada.org/survey2025

Limitations of Liability
If for any reason, the Contest is not capable of running as planned, DMRFC reserves the right at its sole discretion to cancel, terminate, modify, or suspend the Contest. Except as expressly warranted herein, the Prizes are provided to winners as is without further warranty of any kind.

IN NO EVENT SHALL CONTEST ENTITIES BE RESPONSIBLE HEREUNDER FOR ANY INCIDENTAL, CONSEQUENTIAL, SPECIAL, OR INDIRECT DAMAGES, EVEN IF CONTEST ENTITIES HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
